Health Benefits

If you are under 19 and in full-time education then you are entitled to free health care under the NHS.
Once you reach 19, you will have to apply for help with health costs by filling in the HC1 form, available from Ask.
The HC1 form can be used to claim help paying for:

  • NHS Prescriptions
  • NHS Dental Treatment
  • Sight tests, glasses and contact lenses
  • Travel to hospitals for NHS treatments
  • NHS Wigs and Fabric Support Tights.
    Your can fill in this form at any time and do not have to wait until you recieve treatment. Any of our advisors can advise you on how to fill in the form.
    If you have already paid for treatment and you think you are eligible for help, you can claim a refund and for this you will need to:
  • Get a receipt for the treatment you have received
  • Complete an HC5 form which is available from Ask
  • Complete an HC1 form if you haven't already done so
    A claim for all of the above (excluding a sight test) must be made within three months of the date you paid for your treatment. A claim for a sight test must be received within 14 days of the test.
    Make sure your receipt is received within the correct time limit even if you have nor completed the other forms as these can be sent later.
